There are three most important things which needs to be made just about right to get a successful scan: light, focus, and aim.

Light: the more the better. Reflections are a very bad thing so try to avoid them. If you are a lucky owner of a device with built in flash, use it. More often than not flash helps, just be careful with the reflections.

Focus: i.e. how sharp the scanned card name is. The sharper the better, obviously, so it may make sense to increase your normal scanning distance in order to improve focus. Trial and error is the best approach here.

Aim: probably the most important of all three, and perhaps the most difficult to master. Ideal scan is one in which card name text starts in the top left corner. Make sure you cut off the top border of the card. As simple as that, but it's easier said than done, I know.

Practice makes master, I know people who are skilled enough to scan on iPods and iPads which is theoretically impossible.
